Our skilled nursing services are provided by licensed healthcare professionals and are designed to support residents with medical needs requiring ongoing clinical oversight. Care is delivered in accordance with physician orders and individualized care plans.

Comprehensive Care Services

Pico Rivera Healthcare Center offers skilled nursing care focused on medical monitoring, treatment support, and assistance with daily needs. Our interdisciplinary team works collaboratively to ensure care is delivered safely, consistently, and in alignment with each resident’s plan of care.

Services are available based on individual medical needs and physician direction.

Skilled Nursing Services Include

24-Hour Licensed Nursing Care

Continuous nursing support provided by Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), and Certified Nursing Assistants (CNAs).

Medication Management

Administration and monitoring of medications as prescribed by a physician.

Wound Care

Assessment and treatment of wounds in accordance with clinical protocols and physician orders.

Respiratory Care

Support for residents with respiratory needs, including monitoring and treatments as ordered.

IV Therapy

Intravenous therapies administered under physician direction.

Pain Management Support

Clinical monitoring and support for pain management as part of an individualized care plan.

Post-Operative Care

Skilled nursing support following surgery, based on discharge instructions and physician orders.

Social Services & Care Coordination

Assistance with care planning, communication, and coordination of services during a resident’s stay.
